NSS monthly Evaluation Form: How to Print your NSS Evaluation Form

The National Service Scheme (NSS) as part of their way of accessing/monitoring your response (efficiency) to work, they have added a monthly report form /evaluation form that has to be filled out every month…..

NSS Personnel will be paid by what has been filled on the card by your supervisor as instructed by the Secretariat.

Every month throughout your Service year, a monthly report form has to be filled and submitted to your District/Municipal/Metropolitan for final approval before the Secretariat can pay your monthly allowance.

The monthly report form has been structured into three (3) parts.

PART 1, PART 2 & PART 3.

PART 1 has to be completed by the Personnel.

PART 2 is to be completed by the Supervising officer/Headteacher.

PART 3 will be completed by the Director of NSS at the District/Municipal/Metropolitan level.

WHAT IS ON THE NSS MONTHLY EVALUATION FORM?

On the NSS monthly Evaluation Form, you will find your name, NSS number, email, name of institution, your phone number, District/Municipal/Metropolitan, Region, Month/Year, the signature of personnel column, and e-zwich number in PART 1

PART 2 has the name of the organization/school, Title/Rank, area to provide a phone number, name of supervisor, email address, reporting month, Total number of working days, and area to access your punctuality.

The last part, PART 3 is meant for the NSS Director.

HOW TO PRINT NSS EVALUATION FORM

When the Evaluation forms are ready in the portal, you can access them. However, it if is not ready or updated, you CANNOT access it. Let us be guided in order not to be panicking.

Action 1: First of all kindly log in to the authorized website of NSS via https://portal.nss.gov.gh/sign-in and proceed.

Action 2: Log in with your NSS registered credentials (email and password used during registration).

Action 3: Confirm its accuracy before you can tap “sign in”.

Action 4: When the dashboard opens, kindly locate the home button at your far top right area and click it.

Action 5: Kindly glance through the options there and select “Your allawa”.

Action 5: Wait for another menu to display. It will show a button that has print evaluation form.

Action 6: Print and keep a copy.

Kindly make sure that you submit the form on time to avoid payment challenges when the Scheme commence payment of the NSS allowance.
